I'm running B70, and these were the steps to fix this issue: 1. Invoke QS 2. CMD + OPT + S (or click on your gear icon, go to plugins, then shelf) 3. This will bring up a shelf window 4. Click on desired item 5. Delete. This may have been what every one else was referring to. I included a screen shot.
