Could you be triggering your activation shortcut twice somehow? Maybe you customized it to something unusual and also activated modifier-only activation, and did it such a way that doing the one also somehow triggers the other? Or perhaps you are using some other keyboard activated shortcut application that is causing interference?
Also, check to make sure that you don't have two instances of Quicksilver running at once. If somehow you have managed to do that, they will interfere with each other.
