Are you using the Mac App Store version of 1Password?
We're now hoping to improve the plugin to make it even easier to integrate
with 1Password 4/5.

If you are using the Mac App Store, can you open up Terminal.app and type
this command:

printf ~/Library/Containers/*.com.agilebits.onepassword-osx-helper

Then tell us what you get as the output.
