New formatting for BLFS

Manuel has sent me a minor update to the rendering of the BLFS html that 
 he made for the LFS html at Gerard's request.  An example of the 
changed format is at:

The change requires the addition of a line to the xml in every package 
(ugh).  The change is:

[bridgehead renderas="sect3"]Short Descriptions[/bridgehead]
[?dbfo list-presentation="list"?]
[?dbhtml list-presentation="table"?]  <--  Added line
Its possible that this could be done with a sed.  What do you think?
