This was my slipup. It was supposed to be moved to Required untill I had time to build up another LFS and test the patch. Others have done that for me now.

It requires a patch similar to the OOo_1.1.4-no_pam-1.patch except that it also has to remove a big chunk of code from the configure script (so it doesn't check for PAM and fail). There isn't a --disable-pam option, unfortunately. The patch should still be in the archives somewhere from when I was playing with OOo2 a while back; I don't have access to it at the moment to attach it.

It is optional (with the patch) in the same way as it's optional for OOo_1.1.4 in the stable branch.

Yes, it'll be in the book momentarily. The patch is in the patches repo already. First round of changes on the way. Unfortunately, it looks like the assert errors are a glibc problem in both 2.3.4 and 2.3.5. I'm still investigating...and that doesn't explain why this works in my case as I do _not_ have the patch in my glibc....there are others, but nothing related to tls or dlopen.
