I can easily think of a few things: corrupted or damaged files, Jaws seems to get more sluggish as you use it, even with your best efforts, jaws says "graphic 0" for every bit map you land on, settings are not being remembered correctly, even if you set them the way you want, highlight does not track correctly, or worse, at all.
