I would assume that if you open the configuration manager (Jaws key + number row 6) while in outlook express, then go to the set options menu (Alt + S), then move down to synthesizer options, and open that dialogue, if you tab to the punctuation level combo box, change it to whatever, tab to Ok, press space, then press Ctrl + S to save the application specific config file, it should then be specific to application as such.
