It is generally built into the microsoft narrator that OSes like XP have, and it also provides voices etc. for some other apps.

The simplest way to check it out is to go to programs, accessories, accessibility, narrator, or hit the windows key + U key combo to automatically start it.

I honestly just use it as a form of final backup if other things go wrong, or else games like kitchensincs use it's voices for game play etc.
