I have an autorun.inf file thereon with the following in it:
[autorun]
open=nvda.bat
Action=Start NVDA
icon=nvda\nvda.exe
shell\NVDA="Start NVDA"
shell\NVDA\command=nvda.bat
In the nvda.bat file which is also in the root of the flash drive, the
following lines can be found:
@echo off
start nvda\nvda.exe
The portable version of NVDA is all in a folder called nvda on the flash
drive as well.
This will only work on what I think get called U3 flash drives that support
autorun, and it has to be enabled on the computer as well, and, yes, this
will only work after windows has started up.

It can be set to run off of the flash drive as a form of autorun, but this
will only work with certain flash drives etc., and the one advantage is
that
it literally runs off of the flash drive so doesn't need any real
installation or even copying off of the flash onto the computer itsself.
Sometimes if autorun is disabled on that computer or type of flash drive,
you might need to try something like windows key + R to bring up run
dialogue, and then typing in something like:
E:\nvda\nvda.exe
This would be assuming that the C: drive was the primary hard drive, D:
was
the CD/DVD rom drive, so the flash drive should show up as E: generally.

I use NVDA as a backup, attempt to use it if JAWS is not working with an
application or web page well, and use it on my kids computer when I need
to
perform some maintenance. Installing the JAWS video drivers on my kids
computers would interfere with their games and probably violate the
strict
