I would suggest using a spyware detection program along with it.  MSE
doesn't seem to do well against Trojans.  I run frequent scans with
malwarebytes.

That question is probably up for debate, and I would say it's partly
dependant on your level of paranoia.
MSE is all I use on my systems, and I haven't had any apparent problems, but
then I'm not a big surfer of the internet.
