Do you live in a climate where there is freezing in the winter?  If so, it is 
extremely likely that water is getting in the gap, freezing, and pushing the 
slab further from the house.

You need to fill and seal this gap with a flexible concrete gap sealer.  First 
of all, use foam rope caulk to fill the gap up to within about 1/4 of an inch 
of the top of the slab.  This foam rope caulk is available in many thicknesses, 
from very small to quite large.  You can even use more than one size compressed 
down in the gap to fill and seal it to support the pourable gap filler that you 
will add later.  If there are open ends, use pieces of cardboard  slipped in 
the ground along the edge of the slab to close off the end of the crack.  Use a 
pourable, self leveling concrete sealer to now fill from the rope caulk up to 
the top of the concrete slab.  This seam sealer is available in caulk gun tubes 
and gallon jugs.  Most home improvement centers sell both of these products.  
Otherwise, a contractor or concrete supply store can sell you this stuff.
