Unless there is something else substantially wrong with the system I don't see any reason to replace it. By 15 years ago they were making quite efficient units assuming yours is one of those. Generally they last a long time especially if they have well maintained.
Unless you don't already have a closed combustion unit and are going to go to one you won't realize much fuel efficiency improvements. A new boiler will cost plenty and repairing the domestic hot water heater won't or shouldn't cost all that much and hold you over until you are truly ready to replace the boiler. If, on the other hand this unit uses a pilot light and an open flue up the chimney you can probably improve efficiency by 25 or 30% which, at today's gas prices should pay back in five or six years maybe a little more depending on your heating season. You shouldn't need any significant changes to your system just a new boiler and fire system unless you want to use the opportunity to separate off a zone or two which can also improve operating efficiency by leaving unused areas of the building unheated or with reduced heat.
