I've never used one but Paul Harvey swears by his if that means anything.  

I haven't used mine in a good while, but I have a hand irgometer.  That is a 
pedal set up that you run by hand instead of your feet.  It's a great chest and 
shoulder workout and you sure get your aerobic training doe too.
