Basically you'll need a chisel and a hammer.  There should be a bead of 
something, either caulk or glazing compound around the edges of the glass. 
You will need to get that out of the way before you can get out the old 
glass.  It won't take much force with the hammer to get this out of the way. 
Just be careful not to remove more than the sealer holding the glass.  It's 
real easy to dig into the old wood and remove it too.

When you buy the glass you'll also need push points or glazier points.  They 
are a little piece of metal with an arrow point on one end and a little tab 
sticking off of one side.  Once you have the new glass in place, use a few 
points to hold the glass.  The tab on the side is so you can use a chisel to 
seat the point.  When the tab hits the wood of the frame it is seated.  Now 
you can put some new compound in place to seal the new glass.  If you 
haven't worked with caulk before, I would recommend using "glaziers 
compound".  It comes in a can and is much like play dough.  You can roll out 
a piece of it between your hands until it looks like rope.  At that point 
just set it against the side of the glass and press in place.  When you get 
all 4 sides sealed dip your finger in some water and draw it along the 
compound to remove any marks you made while applying it.  Don't use a lot of 
pressure, just enough to drag any bumps out.  You'll get the feel for it as 
you work.
