I pulled the following from the American Thermoform Corp:

I use this stuff and it works quite well.  As a matter of fact, there is a 
cut list stuck to the main support beam for my deck and it has been there 
for more than two years now and the braille is just as readable as it was 
when I stuck it up there.

                                 Braillables

    Our single sheet, adhesive backed, labeling material designed for use
    with Perkins Braillers and other embossing devices. Braille is
    embossed onto these labels and can then be cut to size and will adhere
    to any smooth surface (plastic, metal, wood, etc.).

    This material is ideal for book titles, labeling shelves, canned
    goods, and identifying objects at home, school, or office.

    Each package of Braillabels contains 12 single 8.5x11" sheets. They
    are .11mm thick.

    Embossables are continuous form labeling material developed for
    Braille embossers. Embossables are made of specifically formulated
    plastic with a unique adhesive backing. Plainly speaking, these are
    Braillabels made to be used in continuous form Braille embossers.

    They are packaged in 50 continuous 8.5x11" sheets. For extensive
    production runs, we also offer 500 sheets per box.
