The only thing you need to know for the dado set is the size of your arbor, 
usually 5/8.  Then you can use any brand you choose.  

As far as the insert, you will need the model of your saw.  You can always take 
what you have along and see if they have something that matches.  

You might be able to find the insert online if you know the model number.

  I have a 20 year old Sears table saw with, I think it's called, a tilting 
arbor? I'd like to buy a dato head and appropriate throat plate for it. Do you 
think sears still has those parts and could I maybe get them somewhere else.
