error 289 and no smoking ... yet!

Dear BMElist Subscribers,
I wish to make a general enquiry to determine if anyone has been having 
problems with the Valleylab Force Triad electrosurgical units (ESUs). We 
currently have four ESUs at Royal Melbourne which have been problematic since 
they were purchased in June 2008. One unit has been repaired under warranty 3 
times for 'Error 289', which relates to a RF circuit board.

An incident that is most concerning was last week we had one of the Force 
Triads begin to smoke at the rear of the unit. The ESU was in Theatre at the 
time and in standby mode awaiting use. Tyco Healthcare in response to our 
complaints has replaced both of the ESUs under warranty.

I would like to know if anyone else has been having problems with the Force 
Triads, possibly even raising their concerns to the TGA. Any comments you can 
provide will be most appreciated.
