Did you save a list of what hexadecimal addresses correspond to what points in the program, even with the optimization? If so, you could locate the failure within the failing routine, and add a little more debug output just before that point.
Is some output file still open at that point, or could you open an extra output file for the extra debugging output (preferably by extending some other file, so there will be such a file even if the crash does not occur? If you can consistently get the extra output file, regardless of whether the crash occurs, you can send users some beta test workunits with this extra output enabled but ignored by the validator, then inspect the extra output yourself to see if it mentions anything useful. You may want an extra program to look through such files and delete most or all of those without a crash. For example, if you can identify the pointer used for the crashing operation, and the line of code that crashed, try to make the debug code write that pointer to the extra output file, then flush that output stream, before going on to the line that crashes.
