Ok, I've been looking into this a good bit, and it is not quite so
simple as I thought...

1) The first problem I had was, I created a page, and copied your
sample markups to it. Then saved. Got the results you did. After a
long string of testing, I finally discovered it was the same problem
we've had elsewhere with certain characters being escaped when not
using the edit action. That's on my list to fix.

In other words, look at your source code Marcos and see if you don't
have your ' and " escaped already! This causes the BOLTarg function
to fail. If you reedit and save your page, you can get those replaced
with their proper values.

2) Once I got past that, I discovered there are slashes that not being
stripped, which also causes the BOLTargs function to fail. This can
be fixed by changing line 424 of markups.php from

                $attrs = trim($attrs);
to

                $attrs = trim(BOLTstripslashes($attrs));

This will solve most of the problems. However, if you have Blue's problem:

[[page|label|title="some'thing"]]

The output still becomes

title='some'thing'

Which fails in the final html rendered. This can be fixed using Blues
suggestion of changing line 429 or so from

                        $attr .= " $f='$v' ";
to
                        $attr .= " $f=\"" . htmlentities($v, ENT_COMPAT,
'UTF-8') . "\" ";

With this last change, I seem to get the desired output on my test
server of the three examples posted by Blues and Marcos.

Note: If you create a new page and just copy the text to it, you will
still get garbled titles until you edit and resave the page. That
resets the underlying text. This is high on my list of bugs to fix.
But it is not done yet. Let me know if the solutions above work ok for
you, otherwise. They will be in the next release.

Cheers,
Dan

P.S. Blues, I did not yet escape the args of the styles, tables or
uploads function as you suggested, not having noticed this being a
problem yet. Though we can easily add later. If it does become
necessary, however, I'm inclined to move all the fixes to the BOLTargs
function, rather than to all the individual places that use it. Or at
least, make it an optional parameter so we can decide when and where
to do it. I have a big hankering to redesign the BOLTargs function
anyway, but hesitant to do it just now as it could have VERY wide
implications.
