A friend of mine feeds feral cats...BUT, she also traps them and 
spays/neuters them and gives them their basic vaccinations with her own money. 
The vet 
notches an ear once they have been spayed/neutered so she knows which ones are 
already done. When she first started, there were kittens ALL THE TIME...but now 
(8 years later) a litter is pretty rare.
