The "Documenting Libraries" section has info for new authors... I'm not sure
quite what you're looking for.

You can apply it to any number of files. The problems with Doxygen are mainly
problems with the Doxygen XML format itself, that cost me a _lot_ of time to
deal with properly. From the user side, it seems to work reasonably well.

Some do, most don't. Newer libraries seem to be using BoostBook more, but
libraries that predated BoostBook (most of Boost) haven't been converted
except for a few cases.

No, it means running it through an XSLT processor like xsltproc.

Nope, it's not easy to get started. It's mostly my fault (I should at least
have easy-to-install packages for these things), and partly the fault of the
inherent complexity of the project.
I think the "easy way" is really to use the bjam toolset for BoostBook; it
takes care of most of the gory details. The setup is detailed in the online
documentation.
Or, of course, you can wait until I get my act together and make an easy
installer for your platform. But be warned that I'm ludicrously busy right
now and won't get to it for a while.
