Why should anyone have to manage choosing between = and +?
Why not just something equivalent to what TeX does?

I think we already have that in [section/h1], [h2], [h3], [h4],...
So why should we have to do any extra work? Let the formatting engine
take care of the numbering.

This is all part of the job of the tool that translates the DocBook
into its final format (HTML, PDF, etc.)
