When the header names are different, there's no such error. Isn't this
a bug?
When the last change to lookup.xsl:
http://cvs.sourceforge.net/viewcvs.py/boost/boost/tools/boostbook/xsl/lookup.xsl?r1=1.6&r2=1.7
is removed, the problem goes away.
