Ouch! Comments are terminated by the first ']' So, that is why you see
the ") ]".

Hmmm... really? no ") ]" after the comment? I tried it and
I see the ") ]".

I suggest we write it as:

[/ dist_reference.qbk
   Copyright 2006 John Maddock and Paul A. Bristow.
   Distributed under the Boost Software License, Version 1.0.
   (See accompanying file LICENSE_1_0.txt or copy at
   http://www.boost.org/LICENSE_1_0.txt)
]

i.e. No squares enclosing http://www.boost.org/LICENSE_1_0.txt.
Actually, that too is a quickbook markup that indicates a link.
It does not have any meaning in comments. Sorry for the confusion.
I know I was the one who copy/pasted this :(

Yep. It's supposed to make it easy to write code in between
paragraphs.
