I disagree.  Lack of regard for those issues is a big reason we don't
have lots of powerful C++ source-code processors and better C++
editors today.  These are not emacs-specific concerns, but rather
concerns relevant to any editor (or, I daresay, system that's going to
process quickbook).

a. Humans don't do well with context-dependent syntax either

b. Having syntax highlighting and auto-formatting are key ways to make
   things as easy as possible for humans.
