If it became an industry requirement, there would be a lot of pissed off engineers that would have to do some work is what I feel it boils down to. If ISP's were to become accountable via fines, I'm sure everyone would take a "not in my backyard" approach and work to minimize/stop these issues. But since no one is forcing anyone's hand, most seem to have the attitude of doing nothing and just complaining about it. Then you have those who profit off of the bandwidth being used. And yes as shameful as it may be to some or as shocking as someone could try to pretend it is, how many upstreams are willing to lose revenue when someone maxes out their connection as opposed to tacking on a charge because X_Client exceeded their limits. "Well John, we know they only went over their quota because their network was infested with malware/scumware/whateverware. Let's not charge them for this."
