How do I actually review the commands? I always hit enter when I get this dialogue and goes directly to the file. QT instructions please.

The first question is "Is this a braille or text document?" Choose "text". The second question asks "Translate into braille?" Choose your answer. The third question is "Use line or paragraph style when opening the document?" This is the important question! If you choose "Paragraph" it will strip all the single, hard carriage returns. If you want them to stay, choose "Line". Now, if you choose "Paragraph" when closing, it does not add any new hard carriage returns. If, on the other hand, you choose Line, you get a hard carriage return at the end of every line. In short, to keep things as they are, press: T, N, L, P.
