Just to add to what Brian said about trying the reset on the PK.

hard reset: 4-5-6 with C3C4
If that fails, try a C1C4, this puts the system into deep sleep or 
hibernation mode which wipes the KS disk

If both of the above fails and your card is still not being read, most 
likely you have a dead card
