Hello Homero,

If you receive talking books from the National Library Service, Bookshare can 
verify your disability with them.

Otherwise, there is a proof of disability form which can be downloaded from the 
Bookshare site, and signed by a qualified professional.  That form can be faxed 
or mailed to Bookshare.
