I don't remember if this has been answered, but if you mean having page numbers 
when you ink-print the Braille document, then press ENTER with BACKSPACE with L 
(READ with CTRL with L) while you're in the document and SPACE to the fifth 
item "Print page numbers?", type Y for yes, and hit ENTER.
The next prompt will ask for the position of the page number on the ink-printed 
page.  You must type two letters.  First type T for top or B for bottom, then R 
for right, L for left or C for center.

If you mean a heading, like a title, on top of each page, now that's something 
you need to do manually for every page.  You can jump from page to page by 
pressing SPACE with dots 1-2-6, then SPACE with dot 4 (FUNCTION with DOWN 
ARROW) to move forward, or SPACE with dots 1-2-6, then SPACE with dot 1 
(FUNCTION with UP ARROW) to move back.  I'm not sure if the QT has separate 
PAGE UP and PAGE DOWN keys, but these keystrokes will do the same function.

If you're going to place page headings, as in titles, then you might need to 
adjust the top margin.  Note that if you're ink-printing the Braille document, 
then you should configure that option in the Page Settings list under the 
Ink-Print Settings: ENTER with P (READ with CTRL with B, then P), and not the 
Page Settings listed in the Format Menu after the Ink-Print Settings.
