I will answer two of your questions.

Yes, I use my Braillenote BT while I stand up.  It is held in the case in
such a way that it would be difficult for it to fall out.

I have an 18 cell BT and don't have any problems with it.  I did not want to
spend the extra money to purchase the 32 cell.  That may be done at some
point in the future.  I understand they can upgrade the 18 cell to the 32
cell for the difference in cost.
