To write a slash, use dot 4, followed by dots 3-4. Although there are some
instances where the dot 4 is not necessary, I find it easier to put in the dot
4 every time I do a slash; that way, I don't have to worry about translation
error. For Terri Pannett, the only time I have seen the slash written as dots
4-5-6 followed by dots 3-4 is when it was written in the Nemeth Code for
Mathematics and Science Notation used in the U.S. The Literary Braille sign
for the slash is dot 4, followed by dots 3-4. However, both the Literary
Braille form and the Nemeth form can be used.

Thanks,
Maria
