Your pstat probably stuck, which can cause intermittent overpressure and pop off. Replace the pstat.
Your vacuum breaker can usually be cleaned and new ring—no use replacing the metal part. You should always support the part not being turned, ideally with a wrench instead of pliers. All that supports your boilers is copper tubing and you’ll twist it. A large crescent is useful for this and flexible.
