A few years ago, when Episode I came out, I got a mass email about how to make your Star Wars name. I don't have a copy of that email, but the process is pretty simple.
For your Star Wars first name, take the last 3 letters of your real last name and the first 2 letters of your real first name. If your name were John Doe, that would give you a Star Wars first name of Doejo. For your Star Wars last name, take the first 2 letters of your mother's maiden name (for the security conscious, use your maternal grandmother's maiden name, or your wife's maiden name, or your husband's mother's maiden name) and the first 3 letters of the city in which you were born.
