Lee was killed by a bullet that somehow found its way onto the set of The
Crow.
Jon Eric Hexum (Voyagers) was killed on the set of a film when he placed a
gun filled with blanks to his head.

The thing to remember about blanks is that you are still firing a firearm.
There is an explosion involved and any explosion is going to have some
concussive force. The concussive force is what killed Hexum. The muzzle
velocity of just the air in the barrel is considerable.

I would not doubt that this is true. But on the other hand, one would be
quite foolish to place any explosive device close to ones body.
