I don't know.  Maybe you should write a letter to the editor making that
query, and see what comes of it.  :)

In some ways, things were a lot simpler when people just paid their
doctor what they could and got a pre-penicillin treatment.  Then again,
it's nice that doctors don't get paid in hams and have to experiment
with dosages of the new sulfa drugs on their own children when their
children come down with whooping cough.
