Lots of people who do not want to be kept alive by extraordinary means never make out a "living will" or tell anyone but those closest to them. Most don't think it will take more than that, and most do not actually think that such a situation will ever actually befall them.

I had surgery a couple of weeks ago. The night before the surgery I talked with my wife about this. I do not have a living will. I have not discussed this with anyone else. I never considered that someone else might question that my wife would be lying if something had happened to me.

Which, obviously, it did not!
