The simplest thing is to stop it loading in the first place. It can be started in several ways, but the most common are:
startup folder in your Start Menu. If it is there, simply delete it.
load=loader.exe or run=loader.exe in your c:\windows\win.ini
loader.exe in your c:\autoexec.bat - though this is unlikely
a key in the registry, most likely hkey_local_machine\software\Microsoft\Windows\CurrentVersion\Run


In any of the above (and there will be some variation for various versions of Windows, of course), you can simply delete the entry.
You will probably find that once this is done and the machine rebooted, the file can be deleted. Failing that, start Windows in Safe Mode (keep hitting F8 during startup until it gives you the choice) and delete it that way.
If it is none of those, it could be an extension change in registry, which could be a bit messier.
