Part of the reason for that was/is the exodus of
trained nurses from direct patient care, which is at
least partially influenced by the change in nursing
duties over the past decade, in turn heavily
influenced by the HMO/for-profit hospitals.  Instead
of doing what they had trained for and desired to do -
take care of patients - RNs were/are pushed into
administrative or supervisory positions, while the
hands-on work was relegated to much-lower-paid
"medical assistants" (who for the most part are, I
think, dedicated, but woefully undereducated in real
medicine).  I know a number of RNs who are now
medical-legal assistants or have found some other
outlet that utilizes at least some of their training,
because they felt that they were no longer able to
follow their vocation 'as their conscience dictated.' 
Others have cut back to part-time, or work longer
hours than paid for to fulfill that 'duty.'
