can you please try to open the extensions/gnash.xpi file directly from
IceCat? Does it create that directory?

I had no extensions/gnash.xpi file, since (I guess) it had already been
installed and then it got deleted. Instead I had the directory tree
shown below under the main icecat directory (not ~).
$ pwd
/usr/local/gnu/icecat-3.0.4-g1/extensions
