Follow-up Comment #1:

As I understand it, the *roff coordinate system has the interesting property
that the vertical location of the origin is not determined until the first
output line is typeset!

That's because an output line is set so that it fits on the page (unless its
height exceeds the page length).

The input can do at least three things to manipulate the height of the output
line.

* Set the type size.
* Set the vertical spacing.
* Reserve space above the output line with negative arguments to the `\x`
escape sequence.

I can't think of any more off the top of my head.  (Some experiments reveal
that the application of vertical space doesn't work quite the way I
expected.)
