Do we want midi output to map voices to channels?
If anybody uses LilyPond to engrave orchestral scores, then there might be
more than 16 voices. There are only 16 channels in each MIDI port. Do any
MIDI players support more than one port?
If we do want one midi channel for each voice, we need some updates to the
examples in user manual section 3.5. (I started, but cannot figure out how
adapt the examples to give the desired midi output with the new system.)
