I can't think of any similarly-realistic case for /outside-staff/ objects,
but the "extra-spacing-width" is introduced in section 4.5.1 of the 
Learning Manual so I'm thinking of a short example for section 4.5.2.

If it weren't a tracked bug, issue 601 would be perfect.  I'll probably
suggest a version of 601 that doesn't collide, but needs a bit of extra
space for readability.
