Can't we please make this continue to work the other way around?  I feel
like that is what just about every existing manual does, so it would be
very bad to break it.  Like, just "pretend" that index entries before an
item were actually written after them.  And, this can happen with every
item, not just the first.  I remember we talked about this long ago ...

At one time, I think index entries after the item didn't work with TeX
(caused extra space in the output).

Anyway, I can't say I've reviewed what C makeinfo does.  Or current
texinfo.tex for that matter.
