I had the same problem with a SIM from Belgium operator Mobistar. Got
myself a new SIM from the store and everything works perfectly.

most intriguing... that's a rather old gemplus sim... i start to wonder if
it is not a simcard-vendor related problem. this sim is also supposedly a
3V sim (the newer o2 one discussed earlier is 1,8V) so it can't be the
voltage.

That's what the new one looks like, yes. Do all the not-working ones have
that kind of rounder chip?

the amount of persuasion needed varies from shop to shop...
I went in and dropped a lenghty sentence containing technical details and

I just tried a newer O2 SIM in the freerunner, and it worked just fine, so
I guess the problem is - at least concerning O2 in Germany - limited to a
little older cards. Note the different chip layout of the 2 SIMs (see
attached files).

Just a quick comment: the debug board will also work if you don't have
the correct numbers in the EEPROM: ttyUSB doesn't care about them anyway,
openocd.cfg should have a list of valid numbers, and dfu-util only needs
the vendor/product ID if there's more than one DFU-capable device.
