In our case the page which was using the nasty evil javascript was in a JSP page which Apache forwards to Tomcat using mod_jk 1.2. This problem occurred frequently and many times a client would spew 3-4 requests for the same page per second. This could continue for many seconds spawning hundreds of requests to Tomcat for the JSP. It is like a mini DOS attack.
