See the last paragraph of RFC 2616, S4.2.
My understanding of that text is that it is the client's responsibility to use
multiple occurrences of a request header field only if it is valid to combine
them with a comma.
As far as httpd: It does combine multiple-occurring fields with a comma without
regards to the particular field (Cookie*, Range, whatever).
