Others have explained well how to get your license, I think.  Just a couple
of notes:

Take that, with the DPS form, in person to one of the registration
offices.  You'll have to be patient (that goes without saying with the DPS
:P), and let them remind themselves on how to do it - it's a rare
occasion.  If your address is in Fort Bend, go to the office in Richmond by
the new prison.  It's fairly dull most of the time, and they've done it
before.

The form says that you certify you regularly operate the radio in each
vehicle listed - they did not physically check my setup - it could be as
simple as keeping an HT in your car.

If you have any remaining 'balance' on your current plate registration, it
will not count toward your amateur plate registration, and the expiration
date will reset to the month you apply for the amateur plates.  So, it may
be 'cheaper' to you to wait until closer to your current plate
expiration month.
