As for the wording on the copy....the application says...
"You must submit a copy of your Mobile Amateur Radio License issued
under guidelines set by the
Federal Communications Commission (FCC) with this application."
Nothing in that statement says true copy, certified copy or anything
like that. I have always made a copy of my license and turned it in. You
can always ask them to send it to the state and let them decide if it is
valid.
The form says...
"You may obtain plates for more than one vehicle titled in your name
that is equipped with mobile amateur radio equipment. License plates
will be issued with the same official amateur call letters to each
vehicle registered by the same owner."
There is actually no limit to the number. It used to say on the form 6
vehicles, that has been removed. I have had 4 in days past. Now, only
two vehicles with them.
The interesting thing is that most officers do not know how to read the
return on the tag. They assume that the first listing is the most
current vehicle with the plate. Some see a list of multiple vehicles and
are confused. I received a warning ticket once while driving the blue
vehicle and the description they grabbed from the return was for the tan
vehicle. Had it been a real ticket, would have easily got out of it as
the vehicle was not that which I would have been ticketed in. After all
is said and done, I would get out of the vehicle and take a picture of
my car with the DPS or police car in the background. Picture is worth a
thousand words ;-).
Once I was stopped in Houston by HPD. I pulled over, the officer walked
up to me (I never met him or had seen him before), he said "sorry, did
not realize who you were, have a nice day and walked back to his car".
To this day, I am still puzzled by that stop. Only thing I could figure
is that he saw 3-4 cars with the same plate listed on the return after
he stopped me and figured I must be important. LOL
By the way.... should you also be a handicapped ham, you can get the
plates with the handicap symbol on them by also attaching form VTR-214.

A third car with eztags would really freek them I expect.

Anyhow, you do not take it to DPS. DPS only processes driver's licenses.
You will take the form to your county tax collector's office. They will
process it, have the plates made and then you call them in about a month
and ask if your plates are in. Ask where they are. They could be at the
downtown office and you can ask for them to be sent to any outlying tax
collectors office.
Once a year, you will get a notice to renew the specialized plate. This
is not the paying the registration fee and getting the window sticker.
This is only that you are keeping the specialized plate alive. It used
to cost $2, for the last 3-4-5 years, it has been free. But you do have
to send the notice back. Strange, They send it, you tear off the reply
part, send it back and no money is involved. Best thing the state ever

I had a car totaled, took plates off old car, put them on the new car at
the dealer and left them with the form and copy of my license. In a few
weeks the registration came and put the sticker on the windshield.
Be sure to get replacement plates if you ever sell the car. Otherwise
the other person will be driving around with your ham plates and you
cannot do anything about it. When you get the replacement plates (like
$5), you will be given your plates to keep. Also, if you buy a used car,
you can go in with your ham plates, register the car and you walk out
with the registration and sticker for the ham plates right then. Did
that on a car we bought used.
