The user experience we're going for is that the blind user can have an idea that when they boot the live CD, it will come up speaking. One way to accomplish this is to create a GRUB entry that provides some audible cue when it is selected (e.g., it beeps when you arrow to it, but that requires a patch: https://savannah.gnu.org/bugs/?func=detailitem&item_id=14661). Failing audible cues, we can put the screen reader option at an edge of the list. With this, the user can press the arrow key repeatedly and be somewhat sure they are at the edge of the GRUB menu before pressing return. By placing the screen reader option at the top or the bottom of the list, we can let the blind user select the screen reader option. So, if "Boot from Hard Disk" has to be at the bottom of the list, would it be possible to move the screen reader to the top of the list, the magnifier second, and then make the default selection be the third entry?
